When Alex’s (Erica Durance) brother Luke Reid (Tyler Hynes) is admitted to Hope Zion Hospital in critical condition, Joel (Daniel Gillies) and Charlie (Michael Shanks) must work together to keep him alive while keeping his admittance a secret from Alex. Meanwhile, Alex figures out a way to remove a tumour from a patient pregnant with twins, while Maggie (Julia Taylor Ross) is thrown off her game by the patient’s gorgeous OB/GYN.
Alex (Erica Durance) and Maggie (Julia Taylor Ross), with the help of gorgeous Obstetrician/Gynecologist Dr. Kalfas, must remove a tumour from a patient who is pregnant with twins, but encounter complications when they discover that the tumour is uncomfortably close to the uterus. Throughout the day Maggie is thrown off her game by Dr. Kalfas’s charm. She seeks Gavin’s (Kristopher Turner) professional help in dealing with him, but as the day goes on, the two of them discover there’s more to the OB/GYN than meets the eye. As Alex performs her surgery, Joel (Daniel Gillies) receives news that her brother Luke (guest star Tyler Hynes, WAREHOUSE 13) is in critical condition after a 40-foot fall. When Luke arrives at Hope Zion, Joel works with Charlie (Michael Shanks) to help Luke, who has a metal pole protruding from his leg, all the while trying to keep his admittance a secret from Alex. As unanticipated, complications arise and Charlie is forced to deal with Luke’s spirit, who’s less than easy to talk to – and whose presence brings all kinds of emotions to the surface, eventually causing Charlie to admit his biggest secret to Alex.

Alex (Erica Durance) risks a dangerous surgery on a seemingly inoperable tumour to save a young boy’s life. Meanwhile, Joel (Daniel Gillies) struggles with the challenges of being Chief of Surgery as Charlie (Michael Shanks) struggles with the spirit of Alex’s dead brother, Luke (Tyler Hynes).
A young boy and his father have driven all the way from Winnipeg to Hope Zion Hospital to ask Alex (Erica Durance) to operate on the boy’s extremely advanced and highly malignant tumour. Alex is determined to help them, but the other surgeons are hesitant to perform such a dangerous and seemingly unrealistic operation. Alex is unwilling to give up, and rallies the troops to attempt the impossible. While Alex focuses on her surgery, Charlie (Michael Shanks) is distracted by the spirit of Alex’s brother, Luke (guest star Tyler Hynes, WAREHOUSE 13), who can’t move on until Alex lets go of Luke – and Charlie lets go of Alex. Meanwhile, Joel (Daniel Gillies) struggles with the many challenges of being Chief of Surgery – and between fighting with Shahir (Huse Madhavji) over a controversial Parkinson’s study, trying to keep the budget in check, and arguing with Alex over her dangerous surgery, he has his hands full.
